Your health matters! Every day you take care of others, and Covenant Health wants you to take care of yourself - and to reward you for doing so! The HealthQuest program offers several ways to benefit your health. By choosing to participate in HealthQuest and enrolling through the Personal Health Survey,* you'll receive: A confidential Personal Health Report, sent to you via your individual Personal Health Portal, Savings on the cost of your Covenant Health Plan medical premiums, and Eligibility for other Covenant HealthQuest activities and services throughout the year. (*Only full-time and part-time employees are eligible for participation. Spouses, dependents, occasional employees or Methodist MedicalCenter’s bargaining unit employees should NOT take the survey.) IMPORTANT NOTE! HealthQuest is a voluntary wellness program available to all eligible employees. If you choose to participate, you will be asked to complete the Personal Health Survey that asks a series of questions about your health-related activities and behaviors and whether you have or had certain medical conditions (e.g., cancer, diabetes, or heart disease). You are not required to complete the Personal Health Survey, but if you do, you will receive the benefits discussed above. The information from you will be used to provide you with information to help you understand your current health and potential risks, and may also be used to offer you services through the wellness program, such as a smoking cessation program. You also are encouraged to share your results or concerns with your own doctor. All health information is kept strictly confidential. Covenant Health's Human Resources and Administration will NOT have access to your personal health information. The program is managed solely by Covenant HealthQuest and administered through a third-party company, Applied Health Analytics, which partners with health systems across the country to enhance the health of their communities.
